Key Program Roles

DISID is responsible for the Individualized Budgeting Program. After your budget has been approved, DISID will develop an agreement with your provider(s), if necessary. Once you’ve selected your provider(s), an agreement with DISID may be developed and payments to your selected provider(s) will be submitted on your behalf in collaboration with a Fiscal Intermediary. DISID helps to arrange and pay for the services in your budget through the Fiscal Intermediary. DISID will assist you and your Service Coordinator in determining what existing resources are available for you.

Your DISID Social Worker plays a vital role in the budgeting process. The Social Worker is one of the first persons with whom you will make initial contact and who will be responsible for referring you to the Individualized Budgeting Program.

Your Service Coordinator plays a very important role in your overall program experience. The Service Coordinator is an advocate ho will assist you in identifying your service needs and in developing your budget. The Service Coordinator helps you maximize your budget. This includes taking full advantage of the available services. Your Service Coordinator will also help you make changes to your budget or provide you with information about potential services and supports that may interest you. You may also request that your Service Coordinator be with you for interviews and/or explain and interpret financial statements sent to you monthly. Your Service Coordinator is the critical link between you and DISID.

Catholic Social Services will provide Immediate Response Services for the Individualized Budgeting Program. Immediate Response services include:
