How can I enroll in the Individualized Budgeting Program?

Please contact DISID by phone or fax or in person, or download and fill out an Enrollment Request Form from this website to enroll in the program.

Is it possible to enroll in the Individualized Budgeting Program and continue to receive other services contracted by DISID?

Yes. The Individualized Budgeting Program enables participants and their families to choose the services and supports they want and need. If you are happy with your current services, you may continue to receive those services while supplementing them with the Individualized Budgeting Program as long as there is no duplication.

What is an Individualized Budget?

Your individual budget is the document that tells DISID what payments to make and to whom. It is important that the information in the budget is up-to-date and that you are satisfied with the supports and services that you and your Service Coordinator have included in the budget.

How do I make changes in my Individualized Budgeting Services?

If you are a program participant and are interested in making changes to your services, discuss the changes with your Service Coordinator and s/he will contact DISID once changes are finalized.

How do I make sure the Individualized Budgeting Program meets my needs?

Although the Service Coordinator is responsible for coordinating the process, the Service Coordinator relies heavily on your input and the input of your network of support in order to create a budget that reflects your needs. This is also your opportunity to actively express any unmet needs you may have.
